1、參加本項(xiàng)目磋商的潛在供應(yīng)商必須具備《政府采購(gòu)法》第二十二條規(guī)定的資格條件；

2、參加本項(xiàng)目磋商的潛在供應(yīng)商須在黑龍江省 注冊(cè)登記并經(jīng)審核合格；

3、供應(yīng)商若為生產(chǎn)企業(yè)，必須具有所投產(chǎn)品醫(yī)療器械生產(chǎn)許可證或第一類醫(yī)療器械生產(chǎn)備案憑證；若為代理企業(yè)，必須具有第二類、三類醫(yī)療器械經(jīng)營(yíng)許可證或第二類醫(yī)療器械經(jīng)營(yíng)備案憑證，并提供所投產(chǎn)品醫(yī)療器械生產(chǎn)許可證或第一類醫(yī)療器械生產(chǎn)備案憑證。投標(biāo)產(chǎn)品屬于醫(yī)療器械的，必須具有醫(yī)療器械注冊(cè)證或第一類醫(yī)療器械備案憑證；不作為醫(yī)療器械管理的，需提供國(guó)食藥監(jiān)械頒發(fā)的不作為醫(yī)療器械管理的文件，且以上證照在投標(biāo)有效期內(nèi)均為有效；

4、單位負(fù)責(zé)人為同一人或者存在直接控股、管理關(guān)系的不同供應(yīng)商，不得參加同一合同項(xiàng)下的政府采購(gòu)活動(dòng)。除單一來(lái)源采購(gòu)項(xiàng)目外，為采購(gòu)項(xiàng)目提供整體設(shè)計(jì)、規(guī)范編制或者項(xiàng)目管理、監(jiān)理、檢測(cè)等服務(wù)的供應(yīng)商，不得再參加該采購(gòu)項(xiàng)目的其他采購(gòu)活動(dòng)。

5、擬參加本項(xiàng)目的潛在供應(yīng)商、法定代表人參加政府采購(gòu)活動(dòng)近三年內(nèi)，在經(jīng)營(yíng)活動(dòng)中沒(méi)有重大違法記錄，在“信用中國(guó)”網(wǎng)站( )、中國(guó)執(zhí)行信息公開(kāi)網(wǎng)( /）、中國(guó) ( )等渠道列入失信被執(zhí)行人、重大稅收違法案件當(dāng)事人名單、政府采購(gòu)嚴(yán)重違法失信行為記錄名單及其他不符合相關(guān)法律規(guī)定條件的供應(yīng)商，不得參與本次磋商采購(gòu)活動(dòng)。
